How to use
Giardia lamblia, IgM antibody testing supports the evaluation of suspected giardiasis when stool microscopy or antigen assays are negative or indeterminate despite compatible symptoms. Detection of anti‑Giardia lamblia IgM (also referred to as anti‑Giardia intestinalis IgM) indicates a recent or ongoing infection and helps stage disease as acute. Results are interpreted in conjunction with stool examinations and the clinical presentation rather than used in isolation.
Limitations
This test uses an enzyme-linked immunosorbent assay to identify IgM-class antibodies to Giardia intestinalis in human serum. These antibodies emerge within the first to second week after exposure, bind surface antigens of the parasite, and initiate downstream immune responses aimed at pathogen clearance. Detection of IgM supports an acute-phase giardiasis diagnosis; however, a nonreactive result does not exclude infection because antibody responses vary with timing and host factors. Reporting is qualitative, indicating the presence or absence of anti‑Giardia IgM rather than a quantitative value.

| Indications | Assessment of suspected giardiasis when routine stool examinations or antigen tests are negative or equivocal despite compatible clinical features., Serologic support for diagnosing giardiasis and identifying an acute phase when establishing the diagnosis is challenging. |
