Epstein–Barr virusi (EBV), DNK zondi testi real-time PZR orqali klinik materialda EBV DNK sini aniqlaydi va in situ gibridlanish yordamida to‘qima kesmalarida EBV-kodlangan nuklein kislotalarni histopatologiyaga qo‘shimcha usul sifatida lokalizatsiya qiladi. Ushbu bir-birini to‘ldiruvchi yondashuvlar EBV infeksiyasini aniqlash va EBV bilan bog‘liq patologiyani baholash uchun qo‘llanadi. Klinik qo‘llanishlarga infeksion mononukleozni tashxislash, EBV ni boshqa gerpesvirus infeksiyalaridan va muqobil etiologiyali ekssudativ tonzillofaringitdan farqlashga ko‘maklashish, shuningdek immunosupressiyadagi va transplantatsiya qabul qiluvchilarda EBV reaktivatsiyasini aniqlash kiradi. Amaliyotda ishlatiladigan sinonimlarga EBV DNK testi va EBV in situ gibridlanishi kiradi.

Cheklovlar

Epstein–Barr virusi keng tarqalgan inson gerpesvirusi bo‘lib, asosan B limfotsitlarda replikatsiyalanadi, shuningdek T limfotsitlar va epitelial hujayralarni ham infeksiyalashi mumkin. Yuqish odatda nafas yo‘llari sekretlari orqali sodir bo‘ladi. Birlamchi infeksiya ko‘pincha bolalikda yuzaga keladi va simptomsiz yoki yengil kechadi, o‘smirlar va kattalarda esa isitma, limfadenopatiya, tonzillofaringit va gepatosplenomegaliya bilan xarakterlanuvchi infeksion mononukleoz ko‘rinishida tez-tez namoyon bo‘ladi. Xabar qilingan asoratlar qatoriga gepatit, pnevmoniya, gemolitik anemiya, trombotsitopeniya, aplastik anemiya, taloq yorilishi, miokardit va Giyen–Barre sindromi, ensefalit hamda meningit kabi nevrologik kasalliklar kiradi. Noyob hollarda surunkali faol EBV olti oydan ortiq davom etadi va a’zolar zararlanishi hamda yuqori EBV-spetsifik antitana titrlari bilan kechadi. Birlamchi infeksiyadan so‘ng EBV xotira B hujayralarida umrbod latentlikni o‘rnatadi va immunyetishmovchilik yoki iatrogen immunosupressiya sharoitida reaktivatsiyalanishi mumkin; bu limfoproliferativ buzilishlar va nazofaringeal karsinoma rivojlanishiga hissa qo‘shadi. Nuklein kislota asosidagi aniqlash usullari to‘qimada EBV-kodlangan ketma-ketliklarni vizualizatsiya qilish uchun in situ gibridlanishdan yoki klinik namunalardan EBV genomik nishonlarini amplifikatsiya qilish uchun PZR dan foydalanadi. Nishon DNK aniqlanmasligi noto‘g‘ri namun olish, noto‘g‘ri ishlov berish yoki fiksatsiya, yoki testning analitik sezgirligidan past bo‘lgan nishon nusxalar soni bilan izohlanishi mumkin.
