Qanday foydalanish

Inson xorionik gonadotropinining erkin beta-subbirligi bo‘yicha tekshiruv (erkin beta-hCG; hCG, erkin beta-subbirligi) birinchi va ikkinchi trimestrlarda ona skriningiga homila xromosomal aneuploidiyasi xavfini baholash maqsadida kiritiladi. Natijalar gestatsion trofoblastik kasallikni tashxislash va monitoring qilish hamda davolashga javobni baholash uchun qo‘llanadi. O‘sma markeri sifatida erkin beta-hCG moyakning germinogen o‘smalarini baholash va ularni uzoq muddatli kuzatishda qo‘llab-quvvatlaydi hamda trofoblastik bo‘lmagan malign neoplaziyalar tomonidan hCG ning ektopik ishlab chiqarilishini aniqlashga yordam beradi.

Cheklovlar

Inson xorionik gonadotropini (hCG) 37.5 kDa glikoprotein bo‘lib, umumiy alfa zanjir va gormon-spetsifik beta zanjirdan tashkil topgan. Alfa subbirligi lyuteinlashtiruvchi gormon, follikulani stimullovchi gormon va qalqonsimon bezni stimullovchi gormon bilan umumiy bo‘lib, beta subbirligi biologik spetsifiklikni ta’minlaydi. Homiladorlik davrida sintez asosan sinsitiotrofoblastda amalga oshadi, past darajada gipofizda ham ishlab chiqarilishi mavjud; qon aylanishidagi hCG ning katta qismi jigarda metabolizatsiyalanadi, taxminan 20% si siydik bilan chiqariladi. Homiladorlikning erta davrida (taxminan 2–5-haftalar) qon aylanishidagi erkin beta-hCG tez ko‘tariladi va ko‘p homilalikda nisbatan yuqoriroq bo‘lishi mumkin; 10–11-haftalar atrofida cho‘qqiga chiqadi va keyin pasayadi. Erkin beta-subbirlikni o‘lchash birinchi va ikkinchi trimestr aneuploidiya skrining algoritmlarining tarkibiy qismidir: birinchi trimestrda ona qon zardobidagi yuqori konsentratsiyalar trisomiya 21 xavfi bilan bog‘liq, past konsentratsiyalar esa trisomiya 18 da kuzatilishi mumkin. Erkin beta-hCG gestatsion trofoblastik kasallikni (masalan, gidatidiform mola, xoriokarsinoma) tashxislash va monitoring qilishda ham yordam beradi hamda hCG ni ektopik ishlab chiqaradigan moyak germinogen o‘smalarida va ayrim trofoblastik bo‘lmagan saratonlarda oshishi mumkin; davolashga javobni kuzatish uchun ketma-ket o‘lchovlar qo‘llanadi.
