Qanday foydalanish

To‘qima transglutaminazaga (tTG) qarshi IgG testi (anti‑tTG IgG; tTGA) seliak kasalligi/glutenga sezgir enteropatiyani skrining qilish, tashxisni tasdiqlash va monitoring qilish uchun qo‘llanadi, ayniqsa selektiv IgA tanqisligi bo‘lgan shaxslarda. U dermatitis herpetiformis bo‘yicha diagnostik ishlarni qo‘llab‑quvvatlaydi va davolangan seliak kasalligida glutensiz parhezga rioya qilinishini hamda javobni baholash uchun qo‘llanadi.

To‘qima transglutaminaza seliak kasalligida ustun avtoantigen bo‘lib, endotelial hujayralar, gepatositlar, eritrotsitlar hamda ingichka ichak epiteliysi va shilliq osti qavatida mavjud. Genetik moyilligi bo‘lgan, ko‘pincha HLA‑DQ2/DQ8 ni tashuvchi shaxslarda, tTG tomonidan oziq‑ovqat gliadini fermentativ deamidatsiyasi peptidlarning immunogenligini oshiradi, shilliq qavat yallig‘lanishini, villus atrofiyasini va natijada malabsorbtsiyani rag‘batlantiradi. Seliak kasalligida avtoimmun javob gliadin, endomisiy va to‘qima transglutaminazaga qarshi antitanachalarni o‘z ichiga oladi. Ular orasida anti‑tTG seliak kasalligi uchun yuqori analitik sezgirlik va xoslik ko‘rsatadi. Selektiv IgA tanqisligi seliak kasalligi bo‘lgan bemorlarda ko‘proq uchraydi; bunday sharoitda IgA‑ga asoslangan serologiya soxta‑manfiy natijalar berishi mumkin. Shuning uchun tTG IgG ni o‘lchash IgA tanqisligiga shubha qilinganda afzal serologik yondashuv hisoblanadi va u umumiy zardob IgA bilan birga talqin qilinishi lozim.
